Policy on Lasting Power of Attorney (LPA) Documentation
We have received a number of enquiries regarding Lasting Power of Attorney (LPA) forms.
Please note that Bodey Medical Centre is unable to assist with or complete LPA documentation.
This includes:
- Acting as a certificate provider or witness
- Confirming a patient’s capacity for the purpose of completing an LPA
- Signing or validating LPA forms
LPA documentation is a legal process, not a medical one, and it is outside the scope of NHS general practice.
If you need help completing an LPA, please contact:
- A solicitor, or
- An independent certificate provider familiar with you personally (who is not a relative or directly involved in making the LPA).
If your solicitor or the Office of the Public Guardian requires medical information to support an LPA application, they can write to us formally with your consent, and we will respond as appropriate.
